Click "Save," dump this document somewhere on your computer (maybe just your desktop) fill it out, save it, and then attach it to an email message and send it to Benni at konvicka@earthlink.net. She will update your individual page where the "Information Page" link shows on the bottom of your individual page. If you have trouble opening the Information Page document, try opening it a few times (at least three). Sometimes virus protection software programs initially block links like this.
